Description: Responsible for specified case load in order to strive to improve the quality of life of severely emotionally disabled clients and their families. Completes yearly and as needed diagnostic assessments for clients on caseload, initial assessments for clients new to services and yearly and as needed assessments for other non licensed team member’s case load as assigned in order to coordinate/recommend client care. Completes crisis assessment/documentation as needed to assigned caseload and non licensed team member’s case load in order to ensure client safety and collaborate on appropriate client care. Ensures delivery of needed support services by providing case management to mental health clients. Provides direct advocacy services by procuring shelter, transportation and other services from area agencies. This position has a sign on bonus associated with it.

Position Requirements: A Bachelor’s Degree in Mental Health or a closely related field, with 3 to 6 months experience. LSW required.
